Regarding Certificate of Liability Insurance
Why do I need it?
In the event that someone was to claim injury at one of the booths,
for whatever reason, Aquafest could be named in a lawsuit. Given
that, our insurance policy only covers $1M lawsuit. The vendor is
responsible for anything below that.
What are the requirements for the
certificate?
- Certificate holder is: Lake
Stevens Aquafest, PO Box 670, Lake Stevens, WA 98258
- Certificate holder should be
listed as an additional insured on the policy
- Coverage should be for no less
than $1 million
- Additional information on
completing an ACORD 25 – Certificate of Liability

completed certificate?
There are 4 options:
